The ratio that drives the silhouette
The hourglass impression comes from the ratio between the waist and the hips, not from buttock volume in isolation. Reducing the waist by five centimeters through liposuction of the abdomen, flanks and lower back changes the waist-to-hip ratio as much as adding several hundred milliliters of fat to each buttock, with less risk and shorter recovery. Patients who already have adequate buttock volume but a straight torso get a dramatic change from lipo 360 alone. Patients with a small waist and a flat or square buttock need the fat. The consult test
- Stand sideways in front of a mirror and pull your waist in with your hands. If the resulting silhouette is what you want, lipo is likely enough.
- Stand facing a mirror and press inward at the hip dips. If that alone makes the shape, hip dip transfer without buttock volume may be the plan.
- Ask the surgeon to mark the lipo zones and estimate waist reduction in centimeters. Then ask what the buttock would look like with that waist and no added fat.
- Ask what happens to the harvested fat if you choose lipo only. It is discarded, or in some clinics processed and banked; either is fine. There is no obligation to use it.
The staged option
Some patients do lipo 360 first, live with the result for six months, and then decide about fat transfer on a second trip. The second surgery is shorter because the waist is done, and the decision is made looking at a real result rather than a simulation.
