Why second rounds exist
Before 2018, surgeons injected large volumes in one session, partly into muscle, to deliver a big result in one go. The subcutaneous-only standard caps what a single session can safely place, typically 300 to 800 mL per side depending on the skin envelope, because fat pushed into a full compartment dies or migrates. The fat that survives stretches the skin slightly over six to twelve months, creating room. A second session then adds volume into that room. Where the fat comes from
Round one typically harvests the abdomen, flanks and lower back. Round two has to find fat elsewhere or in regrowth. Common donors are the inner and outer thighs, the upper back and bra line, the arms, and any abdominal regain. Patients who have lost weight since round one may have very little available and are told so honestly. Some surgeons process and cryopreserve excess fat from round one for a later session; results with frozen fat are variable and less predictable than fresh, and not every clinic offers it. Ask what is realistic before booking a flight.
What round two can fix
- Insufficient volume: the most common reason, especially after a conservative first round in a slim patient.
- Asymmetry that became clear once swelling resolved.

What it cannot fix
- Large oil cysts or hard fat necrosis: these need excision or drainage, not more fat on top; see our fat necrosis guide.
- Loose skin: fat under lax skin does not lift it.
- A result built with intramuscular fat by a previous surgeon: the second surgeon works around it, subcutaneously, and cannot undo it.
- Unrealistic expectations from round one: if the first result at twelve months disappointed for reasons of anatomy, more fat has limits.
What round two costs in Colombia

Recovery the second time
Recovery is usually similar to round one but often feels easier: less lipo, shorter anesthesia and a patient who knows the routine. The no-sit rule applies again, garments are worn on the new lipo zones, and lymphatic massage is repeated. Fat survival in round two is often reported as slightly better because the recipient bed already has a rich blood supply from the first graft. Plan 10 to 14 days in Medellín. Colombia's health system was ranked #1 in the Western Hemisphere and #22 globally in the WHO's 2000 World Health Report, and second rounds are routine for its high-volume surgeons.
Verification checklist
- Verify the surgeon on ReTHUS and confirm cirugía plástica is a registered specialty, not just medicina general or a cosmetic diploma.
- Ask which hospital or surgical center the BBL is performed in and confirm it on REPS as habilitated for surgery under general anesthesia. If a hospital claims JCI, check the JCI directory; JCI is hospital-level only.
- Ask the anesthesiologist's name and verify anestesiología on ReTHUS. One anesthesiologist, one patient, the whole case.

Frequently asked questions
How soon can I have a second BBL?
Six to twelve months after the first is standard. Three months is the earliest reasonable window for a small touch-up.
Is a second round cheaper?
Often, with the same surgeon. A new surgeon prices it as a revision.
Where does the fat come from for round two?
Thighs, upper back, arms, and any regained abdominal fat. Slim patients may have very little.
Can frozen fat from round one be used?
Some clinics offer it. Results with cryopreserved fat are less predictable than fresh.
