When to Consider BBL Revision
Not every BBL delivers the results the patient envisioned. Reasons for dissatisfaction include noticeable asymmetry, more fat absorption than expected (resulting in less volume than desired), irregular contours or lumps from fibrosis, and over-correction (too much volume relative to body frame).
The first and most important rule: wait. BBL results take 6–12 months to fully stabilize. What looks like an unsatisfactory result at 3 months may look entirely different at 12 months as swelling resolves and fat settles. Many patients who are concerned at 2–3 months end up pleased with their results at 12 months.
Do not pursue revision surgery until at least 6 months post-original surgery, and ideally 12 months. Operating too early risks damaging healing tissue, increasing scar formation, and producing worse results than waiting would have. Your patience will be rewarded with a more accurate assessment of what actually needs correction.
What Revision Can Correct
Asymmetry: The most common reason for revision. Additional fat transfer to the under-filled side can improve symmetry. Minor asymmetry is normal (no human body is perfectly symmetrical), but significant differences can be addressed.
Volume loss: If more fat was absorbed than expected, additional fat transfer can restore volume. This is essentially a second round of fat grafting.
Fibrosis and irregularities: Hard lumps or uneven texture can be treated with a combination of manual release, ultrasound therapy, and sometimes corrective liposuction or fat grafting.
Over-correction: Reducing volume is more challenging than adding it. Corrective liposuction can reduce over-filled areas, but it must be done carefully to avoid creating new irregularities.
Revision Complexity
Revision surgery is more technically demanding than a primary BBL. The tissue has already been operated on once — there's scar tissue, altered blood supply, and changed anatomy. This means:
- Not every surgeon who performs primary BBLs is equally qualified for revision work
- Ask specifically about the surgeon's revision experience — how many revision cases they've performed and their outcomes
- If your original surgeon is open to revision and you trust them, that continuity can be valuable. If you've lost confidence, seeking a different surgeon for revision is entirely reasonable.
